BOSSIER PARISH SCHOOL BOARD STUDENT ACCIDENT INSURANCE
Bossier Parish School Board Student Accident Insurance provides scheduled benefits on a secondary basis for injuries sustained during supervised interscholastic team sports, as well as school sponsored and supervised cheerleader, dance line, pep squad, band, AFJROTC, vocational education, and archery, fishing, and tumbling club activities.
Treatment must be initiated within 30 days of the date of injury, and the insurer must receive the completed and signed claim form within 90 days of the date of injury. Parents submit completed and signed claim forms direct to the insurer as noted on the form.
